Cardiovascular Disease is the second largest cause of overall disease burden and second most common cause of death after cancer in Scotland.
Many people do not know they are living with CVD risk factors and a key objective of the Scottish Governments Cardiovascular Risk Factors Programme is to optimise early intervention and care to therefore reduce patients’ risk of developing CVD.
Patients eligible for checks are those aged between 34 & 60 and if they meet the following conditions:
- Have not received a full bundle of CVD checks in the last 3 years.
- Blood pressure or Blood tests for cholesterol or sugar levels
- Smoking / Alcohol status
- Height / Weight / BMI / waist measurement
If you have been offered an appointment with our Health Care Assistant for a CVD assessment as part of the Cardiovascular Risk Prevention Programme, it would be useful if you could complete the CVD assessment questionnaire and calculate your Diabetic Risk Score. The Risk Score can be done at https://www.preventdiabetes.co.uk/diabetes-risk-calculator/ If you cannot print off the questionnaire, just write down your answers and bring it along to your appointment.
We will perform your blood pressure, check your weight and perform bloods to check your cholesterol level during this appointment. Depending on your Diabetic Risk Score, we may also check to make sure that you are not diabetic.
