We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care.
If we call you and you would like someone else to speak on your behalf, we will need your verbal permission before we can give out any information unless there is an active Power of Attorney or Third party consent form. If you have a Power of Attorney, please give a copy to the practice so we can note this in your records. You can fill out our Third party consent form here.
This information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like to access your records, you will find the information here. Please note, these requests can take up to 30 days to fulfil.
