We are delighted to introduce our Primary Care Mental Health team. This is a new service being offered within the Practice. To get an appointment with the team, your GP will refer you if they feel it would be beneficial to you.
You will be asked to complete some questionnaires so the team can assess you and provide you with the best support possible. Please complete these before your appointment.
