FIT NOTES – WE NO LONGER EMAIL SICK LINES. Sick lines will be available for collection 24 hrs from the start date of new sick line. please note these lines cannot be collected before 4pm.  Do not turn up before 4pm as the lines are not signed until later in the afternoon

Please only use this form if you ALREADY have a sick line from us and have spoken to one of the GPs. This form is only for extending a current sickline. If you do not have a sick line, but feel you require one, please call us between 8-9am for an on the day appointment.
