Due to a reduction in our list size, we are able to accept registrations for 200 new patients on to our list. Once these 200 places have been filled, our list will close again. Do not turn up at the Practice unless you have arranged a registration appointment. We anticipate that the spaces on our list will go quickly and we need to ensure we have staff available to deal with these.
To register, please –
- Ensure you are living within our boundary area – inc- Bogbain, Balloch, Castle hill, Croy, Culloden, Dalcross, Milton of Culloden, Culloden moor, Smithton, Stratton, Tornagrain and Westhill.
- Complete a registration form for every person wishing to register Application form. As well as a new patient questionnaire Adult questionnaire. or Under 16 questionnaire.
- Make sure you have the correct ID for everyone, where appropriate. Photo ID and proof of address dated within the last three months.
- Contact the Practice by ‘phoning 01463793777 and ask for a registration appointment.
- Attend the registration appointment with completed forms and ID. Our reception desk is on the left as you enter the building.
As per the agreement we have with Culloden Surgery, we alternate registering patients each week. Our Weeks for intake this Month are :
