PLEASE READ THE BELOW BEFORE APPLYING TO REGISTER
Thank you for taking an interest in registering with Culloden Medical Practice. As of 04/09/2023 we will be reopening our list to taking new registrations, we feel our patient numbers are back at a level to accommodate new patients and still provide safe care.
As per the agreement we have with Culloden Surgery, we alternate registering patients each week. We will be taking registrations the week of 11/12/2023 and 25/12/2023 this month. Please ensure that you fit the above criteria before applying to register.
If you are registering with us, please fully complete the registration form and new patient questionnaire. To submit your registration, please email your forms to [email protected]. Make sure that you email your forms with a form of photo ID (e.g. passport) and proof of address (e.g. utility bill). You should be registered on our system within 1 week – we will only be in touch if there is a problem with your registration or if we require more information.
If you are already registered with us and have just had a newborn you can register them with us. Their birth must be registered at the Registry Office. Once you have registered their birth, you will be given a “white card” with your baby’s details on it. We require this card to get the baby registered on our system. Once your baby is registered, we can arrange appointments for their 6 week check.
If you are staying in the area temporarily for up to 3 months, we can register you as a temporary patient if you require medical treatment. We can register you over the phone. If you are staying longer than three months, you would have to register as a permanent patient.
